If you appendix carry, you’ve felt it — the grip of your gun pushing into your gut when you sit, the muzzle end digging into your thigh, the top of the slide printing through your shirt every time you lean forward. The fix everybody points you to is “add a wedge.” But walk into that search and you hit a fork immediately: a traditional foam holster wedge glued to your kydex, or a holster pillow (an adjustable-fill wedge that straps on). I’ve carried both for years. Here’s the honest breakdown of which one wins, and when.

The short answer

  • Traditional wedge = a fixed block of foam, usually stuck to one holster. Cheap, simple, zero adjustment. Great if you’ve dialed in one setup and never change it.
  • Holster pillow = an adjustable-fill wedge with a Velcro strap. You tune the firmness and loft, move it between holsters, and take it off to wash it. Costs a little more; solves the two problems a foam block can’t — fit that changes with your body and your carry position.

Where a traditional wedge is genuinely fine

I’m not going to tell you a foam wedge is useless — it isn’t. If you carry the same holster every day, in the same position, and a fixed amount of foam happens to hit your body right, a stuck-on wedge will quietly do its job for cheap. Plenty of people never need more than that.

The problem is the word “happens.” Foam density is a guess made by someone who’s never met your body. Too soft and it bottoms out; too firm and it trades a dig for a bruise. And the day you change holsters — new gun, summer carry, a different position — you’re peeling foam off kydex and starting over.

Where the pillow pulls ahead

Three things a block of foam physically can’t do:

  1. Tune the firmness to you. Bigger gut, bony hip, all-day desk job — they all want a different loft. With an adjustable-fill pillow you add or pull fill until the grip rolls in and the printing disappears, instead of living with whatever density the factory chose.
  2. Move between holsters. One pillow, every rig. Straps on with Velcro, comes off in a second. You’re buying comfort once, not re-buying foam for every holster.
  3. Come off to wash. It sits against your skin all day in the summer. Pull the fill, wash the cover, done — something you simply can’t do with foam epoxied to a shell.
